INSPECTION REQUEST FORM

Health Facility and Suppression Inspections must be requested by noon (12:00PM) the Thursday PRIOR to the week of your inspection request date.  Your Permit Card and any approved documents must be on site for an inspection to take place. 

School Inspections will be scheduled by the inspector assigned to the School's District or county the project resides in.
Permit Information

Please type in your entire permit number and SELECT the permit from the list. Permit Number should be entered in the format provided P-00XXXXX and the five number permit ID after the zeros. If you do not select your permit from the list, your request may not process properly. If you are unable to locate your permit number, it means your permit has not been issued and you are unable to schedule an inspection
